P-P08 Towards effective analgesia in acute pancreatitis: a systematic review of randomised controlled trials

Abstract Background The optimal analgesic strategy for patients with acute pancreatitis (AP) remains unknown. present systematic review and meta-analysis aims to compare the efficacy of several modalities trialled in AP. Methods A search PubMed, MEDLINE EMBASE was conducted up until June 2021, according PRISMA Guidelines identify all randomised control trials (RCTs) comparing primary outcome measure improvement pain scores as reported on visual analogue scale (VAS) day 0, 1 2. Results Twelve RCTs were identified including 542 patients. Seven trial drugs compared: opiates, non-steroidal anti-inflammatories (NSAIDs), placebo, local anaesthetic, epidural, paracetamol metamizole. weighted single-arm effects estimate showed global VAS across from baseline On inspection, epidural analgesia appears provide greatest within first 24hrs, however at 48hrs it comparable opiates. Within NSAIDs offered similar pain-relief while placebo also equivalence other but then plateaued. Local anaesthetics demonstrated least overall efficacy. opiate non-opiate analgesics 1. significant heterogeneity reporting relatively small datasets per study. Conclusions Given incidence AP there is remarkable paucity level evidence guide management. Epidural administration most effective modality 24hrs are an sparing alternative during 24hrs.
